Nuseeka’s approach to root cause analysis is systematic and meticulous. We follow a seven-step process that includes but is not limited to: problem definition, data collection, causal factor identification, and solution implementation. We also use standard process improvement quality tools to identify the root cause of the problem. The seven steps are listed in the section below.
In this phase of the analysis, we work to clearly define the problem and its impact on the system. It is important to define the problem in a way that is objective and unbiased. This step is crucial because it sets the foundation for the rest of the analysis. A poorly defined problem can lead to incorrect conclusions and ineffective solutions
We strive to ensure that the data collected is relevant, accurate, and sufficient to identify the root cause of the problem. We may collect data through various methods such as interviews, surveys, observations, and document reviews. Once validated, the data is then used in the subsequent steps of the analysis.
In this step, we attempt to identify the causal factors that led to the problem. Causal factors are the underlying reasons or conditions that contributed to the problem. We use various tools such as fishbone diagrams, 5 whys, and fault tree analysis to assist in identifying the problem, not just the effects. As such, It's important to identify all the causal factors to ensure that the root cause is accurately identified.
This step involves identifying the fundamental reason(s) that caused the problem. Root cause identification is a critical step in the process because it helps to ensure that the problem is permanently resolved. It is important to identify the root cause accurately to avoid treating only the symptoms of the problem.
The solution development step involves developing solutions to address the root cause of the problem. We ensure that solutions are specific, measurable, achievable, relevant, and time-bound (SMART). Brainstorming sessions, affinity diagrams, and tree diagrams are some of the tools we use to develop solutions. We involve all stakeholders in the solution development process to ensure that solutions are practical and acceptable.
This step involves implementing the solutions that were developed in the previous step. The solutions should be implemented in a controlled and systematic manner to ensure that they are effective and sustainable. We assist clients in documenting all actions taken to address the problem to ensure that they can be repeated, in the future, if necessary.
This step involves evaluating the effectiveness of the solutions that were implemented. The evaluation can be done through various methods such as performance assessments, experiments, and tests. The results of the evaluation are used to determine if the solutions were effective in addressing the root cause of the problem. If the solutions were not effective, the process is repeated from the beginning
